Provides General Health Services such as prenatal, family planning, high risk pregnancy program, presumptive eligibility, perinatal case management, children with special needs program, health check, well baby check up, immunizations, nutritional counseling, stroke and heart attack prevention program, WIC (women, infants and children), STD/HIV testing and counseling, health education services, health screenings, environmental services, women's health, marriage blood tests, BreasTest/BreasTest & More, mammography, hearing/vision screening, children's dental services, travel clinic, provides ambulance inspections, tobacco prevention program, chronic disease program, adventure based counseling program, tuberculosis screening and pregnancy tests. Services are available to anyone. Cost of services is based on sliding fee scale determined by income.

Business Hours: Monday, Wednesday and Thursday 8:00 a.m. (the first Wednesday of the month - 9:30 a.m.). - 4:30 p.m.; Tuesday 8:00 a.m. - 6:30 p.m. and Friday 8:00 a.m. - 2:30 p.m. Closed from 12:00 p.m. - 1:00 p.m. for lunch.

Primary Care Clinic
(located at the Hall County Health Department)

Northeast Georgia Physicians Group offers Primary Care Clinic services at the Hall County Health Department for adults with chronic illnesses as well as preventive services. Although the Primary Care Clinic is located at the health department, it is not a health department program. This department provides birth and death certificates.

The State of Georgia's birth and death records started in 1919. Only Hall County births and deaths are at this location.

To inquire about births and deaths in other counties and states please call 404-679-4701, 4702 or 4703 for their phone numbers.

Birth Certificates are confidential and are kept in the county of the birth.

For a child's birth certificate: Only natural parents, grandparents or legal guardian can purchase the record.

For an adult: Only natural parents, grandparents, legal guardian, spouse, sibling, or grown child can purchase the record.

To purchase a birth or death certificate:

1. Rush orders can be sent Federal Express using customer's bank card by calling 1-800-255-2414.

2. Request through mail: The search fee of $15 (includes first paper copy, if found) is not refundable. You must send a Money Order for $15 and $5 for each additional copy to: Vital Records, 1290 Athens St., Gainesville, GA 30507.

  • For birth certificate requests, include your relationship to the person you are requesting the record for, their name, date of birth, mother's maiden name, and father's name. A legal, legible picture, i.e. driver's licenses, of the person requesting the birth certificate must be included (copy, do not send original). Your telephone number and a self addressed stamped envelope must also be included.

  • For death certificates, include your relationship to the person you are requesting the record for, their name, place of death, date of death, funeral home (if known), and any other information that would be helpful in locating the record. A legal, legible picture, i.e. driver's licenses, of the person requesting the birth certificate must be included (copy, do not send original). Your telephone number and a self addressed stamped envelope must also be included.

3. Come in person to the Hall County Health Department with your picture I.D. You must have a legal picture I.D., i.e. Driver's License. The search fee of $15 (includes first paper copy, if found) is not refundable. There is a charge of $5 for each additional copy of each birth or death certificate requested. We accept cash, Discover, Master Card or Visa. No personal checks accepted.

Name changes, Paternity Acknowledgment (adding the father's name when the parents are not married), and Legitimations (adding the father's name when the parents were married after the birth of the child), can be done at this location by appointment only.

Hall County Board of Health

The Hall County Board of Health oversees the activities and programs of the Hall County Health Department. A function of the BOH is to determine the health needs and resources of the county by research, collection and evaluations of data pertaining to the health of the community. The BOH is also responsible for developing in cooperation with the DHR, programs, activities, and facilities responsive to the health needs of the county and insuring compliance with rules and regulations of the DHR. The BOH is charged with enforcement of all laws pertaining to health unless the enforcement of those laws is by law the responsibility of another agency.
